The curriculum of NTMC is patterned after the US medical Program. The medical program in NTMC is of 6 years divided into three parts which consists of Pre Med, Basic Science and Clinical/Rotation/Clerkship.
The Clinical Rotation/Internship/Clerkship is of 78 weeks.
The 78 weeks clinical rotation program is completed in affiliated hospitals across different states of U.S like Utah and Maine. For Indian students options for UAE and Nepal are also available.
FIFTH YEAR & PARTIAL SIXTH YEAR
(Will be taught at our teaching hospitals in the U.S. or other countries)
- Electives: (Minimum 3 weeks; Maximum 6 weeks) Total: 30 weeks
- Electives must include 8 additional weeks of medicine (or subspecialties), and 4 additional weeks of surgery.

Course duration for clinical clerkship is 2 years for $7000 per year
There is no fixed time for enrollment into clinical clerkship, it is open throughout the year. Students after enrollment can immediately start their clerkship with the core clinical subjects according to the mandatory weeks necessary to finish the course
The students can complete their clerkship in the partner universities of NTMC, namely University of Maine, Southern University of Utah, State University of New York, Oswego.
However, the Indian Students can pursue their clerkship from UAE and Nepal as well, owing to the fact that the living cost of these countries are comparatively affordable also these countries are less effected by the ongoing global crisis.
For Nepal, the clinical clerkship of 78 weeks must be completed within a time span of 2 years. Wherein the fees for the course is also divided across 2 years which is $7000 per year, which also includes tuition fees, hospital charges for clinical clerkship.
The above mentioned fee structure may vary for other countries.
Students required to complete 6 months of B.S will have to pay additional $3500 which will be conducted in the same country chosen by the student for clerkship.

India is home to the world’s largest youth population. Our young people, if properly taught, will be able to meet the global demand for competent experts, particularly in the healthcare field. While India is striving to raise the number of MBBS seats to fulfil local demand, it will still fall short of meeting the country’s medical needs. This pandemic has taught us that medical professionals are scarce in nearly every country. Medical study in abroad is a boon to Indian students who are unable to gain acceptance in their native country. However, the recently released ‘National Medical Commission (Foreign Medical Graduate Licentiate) Regulations of 2021’ have produced uncertainty and raised questions among Indian students studying or intending to study medicine overseas.
In this article, we attempt to explain why students wishing to study medicine abroad need not be concerned about the new NMC regulations.
The New FMG licentiate regulation was published in the Indian Gazette on November 18, 2021, and went into effect on the same day, posing the following questions:
“Section 4 (a) (i) undergone a course leading to foreign medical degree with minimum duration of 6 years;”
in New Tokyo Medical College, the primary medical qualification to complete 6 years including 78 weeks of clinical rotation/internship/clerkship.
With the new regulations in place, the NMC mandates that the overall period of the foreign medical qualification (theory, practical and clinical training) is similar to MBBS in India, which is governed by “Regulations on Graduate Medical Education 1997, (Amended, 4th November 2019)” and lasts 4.5 years or 54 months.
As stated in Section 4 (a) (i), the PMQ earned from a foreign country must be of 54 months.
Hence, we can conclude that the MD program in New Tokyo Medical College is valid in terms of the above mentioned conditions laid by the new notification and commensurate with the Regulation as well.
The Regulation’s phrasing is highly ambiguous and may be difficult to understand:
2.1: The title of Schedule I Section 2(i) “Duration of course and training in courses leading to PMQ” must be noted.
- The required course structure and clinical subjects for a twelve-month internship are listed in Section 2(I) (a), and (b). The title does not read “after” PMQ, but rather “leading to.” The internship requirement listed above is the clinical training received during the course, not afterwards, as the title implies.
2.2: Section 2 (ii) states “Content of course or Training leading to a primary medical qualification,
- The course and internship or clinical clerkship shall commensurate with the MBBS course of India in accordance with the regulations on Graduate Medical Education, 1997 and subsequent amendments thereto, and shall include the subjects mentioned therein as are duly recognized by the Commission under sub-section (2) of section 61 of the Act;”
- The Regulations on Graduate Medical Education (2019 amendment) updates the curriculum of MBBS for all medical colleges in India from the academic year 2021-2022 to “competency-based learning”, which focuses on “the desired and observable ability in real-life situations”. This means that clinical training will receive greater weight beginning in the first year.
- A clinical internship in the college hospital is also a requirement for students in their third year of study.
- As stated in Section 9.5 “Learner-doctor method of clinical training (Clinical Clerkship),” the clinical training is also referred to as clinical clerkship, internship or junior internship.
The clinical subjects for clerkship of MBBS specified in section 8 table 8 are the same as those listed in Section 2(i)(b) of the FMG licentiate regulations. The clerkship subjects of the PMQ of New Tokyo Medical College are the same as the MBBS syllabus of India.
2.3: Schedule I Section 2 (iii) also reiterates that the entire course, training and internship or clerkship required as part of the PMQ must be completed in the same foreign medical institution and no part of the of medical training shall be done in India or any other country other than country from where the primary medical qualification is obtained, which includes an internship or clinical clerkship.
However, herein it should be taken into account that medical qualifications granted by medical institutions outside India are dealt within a special provision, that is, Section 13(4A) of the act which states according to an advisory issued by Medical Council of India (Now NMC) on 02.09.2020 regarding acceptance of internship of Foreign Medical Graduates wherein it is stated that- “Primary Medical qualification obtained from any Foreign Country would be accepted as valid provided that such medical qualification is recognized for enrolment in that country. It is also stated that internship completed by such graduates must be taken into consideration while processing the applications of foreign medical graduates for grant of registration”.
Furthermore, we can also refer as a precedent,
A writ petition(writ petition no: 14974) filed by Rudraraju Jaswanth & Orsvs Union of India & Orsin the year 2020,wherein the petitioner was a medical student of Central America Health Sciences University, Belize had completed the internship/clerkship for 108 weeks at Man Mohan Memorial Medical College & Teaching Hospital at Nepal on account of low expenses as compared to US, UK, Mexico or UAE, thereby, after completion of the same the University awarded him with the degree of “Doctor Of Medicine” on 03.05.2019, which proves that the Belize Government grants such provision of medical training and internship or clerkship.
The petitioner also passed the screening test through National Board of Examination and were issued a screening test certificate on 13.03.2019 and was granted with a provisional medical registration certificate on the ground that they have completed the course in two different countries.
As mentioned earlier, that according to an advisory issued by NMC on 02.09.2020, if the country from where the PMQ is obtained recognizes the medical training for enrolment in that country then such qualification including the clinical internship shall be taken into consideration for processing of registration application.
- In addition to the above statement, the foreign medical graduates are also governed by the Screening test regulation, 2002, as per section 2(g), if the candidate is desirous to obtain a provisional or permanent registration then they have to pass a screening test.
- According to the Regulation 11 of the Screening Test Regulations, if the candidate has not undergone one year internship then after passing of the screening test the candidate is eligible for an internship in India.
- It also states that if the candidate has already undergone one year internship by an approved institution then that must be taken into consideration.
(The clause for an approved foreign medical institution is already mentioned in the advisory issued on 02.09.2020).
However, after subsequent hearing, an order was issued in favor of the writ petition to NMC and Andhra Pradesh Medical Council stating to grant the medical practitioners their permanent registration.
In the light of the above article, it can be concluded that:
- The course offered by New Tokyo Medical College, Kolonia, Pohnpei , Micronesia is at par with the curriculum standards laid by MCI (now NMC).
- If the country where the PMQ is obtained recognises the course for enrolment in that country and issues a certificate of the medical course then that shall stand valid in case of the Medical Qualification including the Internship/ Clerkship even if it is done partly in two different countries.
- The candidates are also eligible to appear for screening test by National Board of Examination and obtain provisional or permanent registration as is applicable in case of the candidate.
- It is clear that the clinical training obtained during the final year of the Doctor of Medicine program in New Tokyo Medical College is equivalent to the clinical placements/clinical clerkships required for the MBBS course in India, and the regulation does not imply the completion of a separate internship following graduation.
Hence we can conclude, that the students desirous to obtain medical degree from Foreign Universities must not be concerned about the new gazette.
